You're not explicitlying adding vmware-fdm into your image either, correct? There's no longer a need to do so. Auto Deploy 5.1 will add it automatically.
Otherwise, regarding the host profile any chance the profile was originally created from a ESX/ESXi 4.1 host? I've heard of some issues associated with using an older host profile on a newer ESXi machine.
